Please arrive at the physician’s clinic at least 30 minutes before your scheduled appointment time so that you can provide us the necessary paperwork and insurance information to put you into our system.

If previous X-rays, MRIs, or CT scans have been performed in regard to your visit, please bring the actual CD as well as the report, which you may request from the imaging facility. Please bring any pertinent lab results.
If you are being referred from another office, please ensure the referral and last office note are sent to our office prior to your appointment, or you may bring it to the appointment.
No, you do not need a referral to make an appointment unless required by your specific insurance plan. We do accept most insurance providers. Please contact your insurance providers for inquiries.
We are currently accepting new patients. However, for your visit to be covered through your insurance, please contact your insurance provider to understand the terms of your policy.
Deductibles and copays must be paid at the time of your office visit.
We require 48 hours notice for all office visits and EMG/NCV cancellations to avoid cancellation fees
Because we feel you need a thorough understanding of your results, we review these in person at your follow-up visit. We ask that you bring a copy of your CD from the Imaging Facility.
Excluding life-threatening findings, results will be reviewed at your follow-up appointment. To ensure timely review, follow-up appointments may be scheduled at checkout.
Yes, upon request we are able to provide you copies with your test results.

Cash, checks, and all major credit cards are accepted for payment.
If you have insurance, please bring in your card. If you are not insured, we require payment at the time of service. We strive to have all aspects of our practice, including our professional fees, reflect our ethical and rational approach to your care.
If you do not show up for your scheduled appointment and you have not notified us at least 48 hours in advance, you may be subject to cancellation fees.
